What Does a Care Coordinator Do?

Care coordinators (or patient care coordinators) manage the overall health care needs of patients with serious health conditions. They consult with various health care providers to determine appropriate medication and help patients prepare for the best treatment options. They are usually licensed nurses.

In the care coordinator role, you will be responsible for monitoring the implementation and evaluation of your patient’s treatment plan. You will be expected to educate them and help them understand their health problems and required medication. Success in this role will be determined by the overall progress of your patient.

  • Promotes strict adherence to facility policies and procedures
  • Collaborates with multi-disciplinary health care professionals to meet patient needs
  • Processes health care reimbursements
  • Develops transition or discharge plans for patients
  • Establishes good relationships between patients and their family members, hospitals, and other health care providers
  • Conducts case reviews and evaluates treatment plans

Care Coordinator Job Requirements

  • Minimum 2 years of clinical experience required
  • Deep understanding of nursing practices
  • Registered nurse licensure required
  • CPR/BLS certification required
  • Case Management certification preferred
  • Excellent communication and interpersonal skills
  • Bachelor’s degree in Nursing or related field from an accredited nursing school/program
