- Develops transition or discharge plans for patients
- Establishes good relationships between patients and their family members, hospitals, and other health care providers
- Conducts case reviews and evaluates treatment plans
Care Coordinator Job Requirements
- Minimum 2 years of clinical experience required
- Deep understanding of nursing practices
- Registered nurse licensure required
- CPR/BLS certification required
- Case Management certification preferred
- Excellent communication and interpersonal skills
- Bachelor’s degree in Nursing or related field from an accredited nursing school/program
